Intervention Components

Our intervention uses a systematic approach to reduce antimicrobial prescribing. Our protocol is constructed into four key phases: enrollment, data orientation, clinical staff training, and data feedback loops. During enrollment, coordinators or SNF staff lead efforts to promote study engagement and educate on ASP requirements. During Orientation, the SNF is instructed on monthly data upload procedures using the Research Electronic Data Capture (REDCap) software. Clinical Staff Training and Data Feedback Loops are conducted in parallel.  

Data are collected on facility-wide metrics including C. Difficile orders, ESBL producing isolates, and resident-specific features including culture results, antibiotics issued, and clinical symptoms. Autogenerated reports are distributed back to facilities to provide feedback on prescribing trends. Read the steps below to learn more about the training and quality improvement sequence:

Senior team onboarding

Community physician meets with ED, DON, and MD to describe participation expectations. Orientation handout is distributed and discussed. An announcement letter is signed by all three staff.

Resource Documentation

  1. Consent and REDCap training conducted by staff for IP or DON. 
  2. Module 3 groundwork; establish meeting spaces and IT set-up; a conference room is identified.

In-service trainings 

Trainings (10-25 min.) are conducted for the SNF; three training times are provided (i.e. AM, PM, weekends). Training contents include:

Competency Meeting

An initial monthly webinar is hosted remotely. All SNF Prescribers or IPs are invited to join. Case scenarios are presented to test knowledge of appropriate prescribing.

Monthly Webinars 

Monthly webinars based on the prevention collaborative model are continued after Module 4. Webinars are not mandatory, but provide a space for SNF staff to discuss challenges.
